Athletic Requirements

The National Collegiate Athletic Association (NCAA) mandates that all potential student athletes, freshman and transfers have a current Medical Evaluation Form on file prior to participation in intercollegiate athletics. The medical evaluation (pre-entrance physical) form must be completed by a physician within six months of your arrival on campus and returned to the Student Health Center.
In addition, every student athlete must have proof of health insurance that provides coverage for their participation in intercollegiate athletics up to $75,000.The College at Brockport student health insurance policy covers athletically related injuries up to $75,000. A copy of your medical insurance card, front and back must be on file with the Office of Intercollegiate Athletics prior to your participation.
