Measles, Mumps, and Rubella
All students, born during or after 1957, taking six or more credits must provide proof of immunity against measles, mumps, and rubella. Request that your doctor or previous college fax (585) 395-2559 or mail your shot records to the Student Health Center before classes begin or you will be de-registered and not allowed to attend classes. (This is required by New York State Public Health Law #2165.) In addition, the Office of Registration and Records will charge a fee to re-register your classes.
Meningitis Vaccine
New York State Public Health Law also requires post secondary students to understand the risks and recommendations of meningococcal meningitis vaccination. You must submit the date you received the shot or decline the vaccine before classes begin or you will be de-registered from classes. In addition, the registrar will charge a fee to re-register the classes.
Health Insurance
Health insurance is mandatory for all full time undergraduate and graduate students. The charge is entered directly on the student bill; students with comparable coverage may waive the charge.
Medical Evaluation Form
We recommend all students complete the Medical Evaluation Form and return it to the Health Center before classes begin Intercollegiate athletes: All potential athletes MUST submit a completed Medical Evaluation Form that has been completed by their doctor within 6 months from the start of classes.
Brockport Bound is sponsored by the Office of Undergraduate Admissions.
