Mandatory Immunizations, Health Insurance and Medical Form
Measles, Mumps, and Rubella
All new students born on or after January 1, 1957 taking six or more credits must provide proof of immunity to measles, mumps and rubella, as required by New York State Public Health Law #2165. You must request that your doctor or previous college fax (585) 395-2559 or mail your shot records to the Student Health Center before classes begin or you will be de-registered from your classes. In addition, the Registrar will charge a fee to re-register the classes. (This is required by New York State Public Health Law #2165.)
Meningitis Vaccine
New York State Public Health Law also requires post secondary students to understand the risks and recommendations of meningococcal meningitis vaccination. You must submit the date you received the shot or decline the vaccine before classes begin or you will be de-registered from classes. In addition, the registrar will charge a fee to re-register the classes.
Health Insurance
Health insurance is mandatory for all full time undergraduate and graduate students. The charge is entered directly on the student bill; students with comparable coverage may waive the charge.
Medical Evaluation Form
All students should submit the Medical Evaluation Form to the Health Center prior to arrival on campus. It is required to be on file before participating in intercollegiate athletics or receiving allergy shots.
Brockport Bound is sponsored by the Office of Undergraduate Admissions.
