Instructions to Waive
Mandatory Student Health Insurance Plan
Health insurance is mandatory for all full time undergraduate and graduate students enrolled in 12 or more credits. The College has a health insurance plan through Aetna Student Health for the 2008-2009 academic year. All full time students are automatically charged for the plan on their student bill.
If you have health insurance coverage, you may waive the insurance charge by completing an online electronic waiver. Please see the waiver instructions on the reverse side of this card.
The waiver deadline for the fall 2008 semester is September 23, 2008. For new students registering for the first time in January 2009, the waiver deadline is February 23, 2009 for spring 2009. Any student who has not completed the waiver process by that date will be responsible for full payment of the insurance charge.
The insurance charge on your bill can only be removed by completing the online waiver process. Again, if you do not complete the waiver process on or before the dates stated above, you will be responsible for full payment of the insurance charge.
To complete an online waiver have your insurance card in hand, as information contained on your insurance card is required to complete the waiver.
PLEASE NOTE: You must use your Brockport ID number, not your social security number.
- Access Aetna Student Health at www.AetnaStudentHealth.com
- Click on “Student Connection”
- Click on “Find Your School”
- Enter either the policy number (812826) or school name then click on the link.
- Click on “Enroll/Waive” located in column at left.
- Select “Click here to waive from Student Medical Insurance Plan”
- Click on “Continue” and follow the prompts to complete the electronic waiver transaction.
- Keep the confirmation number for your records.
You will be allowed two (2) attempts to complete the online waiver. If, after the second attempt, you have been unsuccessful, or if you have any questions concerning the waiver process or the insurance policy, please contact Aetna at 1-877-623-3335.
