Academic Advisement
Academic advisement is provided by department faculty. Each student is assigned a faculty advisor upon completing the Intent to Major or the Declaration of Major form (click here for an explanation of the difference between these forms; opens in a new browser window). Students should seek advisement from their assigned faculty advisor, who will have a complete file on the student, including the DARS report and the Student Advisement Key number that is needed to register for courses . In the event a student cannot see the assigned faculty advisor, Kim Breslawski, the department's Assistant Advisement Coordinator, can provide advisement. Procedures for Special Course Registration

Note: All form requests must be accompanied by DARS.

Advisement Forms
Advisement forms are useful for tracking progress towards the requirements for your degree and for preparing for meetings with your academic advisor before registering for courses.
If the advisement form you need is not listed above (e.g., you matriculated earlier), you will have to get the form you need in the Business Department at 119 Hartwell Hall.

Add/Drop Forms
- Add Closed Section Request Form (Word document)
Complete this form and attach a copy of your full DARS report to request adding a closed section to your schedule.
- Course Withdrawal Form (opens in new browser window)
Complete this form to withdraw from a course after the drop period.
Transfer Credit Forms
- Notice of Award of Transfer Credit Form (opens in new browser window)
Complete this form to request transfer credit for a course already taken at another college. You must attach the syllabus from the course you took.
- Student Course Approval Form (opens in new browser window)
Complete this form to get approval to transfer course credit for a course that you plan to take at another college. You must attach a syllabus for the course you plan to take.
Waiver Forms
- CIS106 Waiver Form (Word document)
This document explains how to apply for a waiver for CIS106 (End User Computing) and contains one of the forms you must complete and include with supporting documentation. You also have to complete a "Courses Substitution and Waiver Form" that can be obtained from the Business Department (119 Hartwell Hall) or from Registration and Records (201 Rakov Center).

This document explains how to apply for a waiver for ENL308 (Business Writing) and contains the form you must complete.
