Here you will find some of the more frequently used forms including various advisement forms and forms for transfer credit requests, course add requests, course withdrawal requests, CIS106 and ENL308 waiver requests, and declaration/change of major.
Advisement Forms
Advisement forms are useful for tracking progress towards the requirements for your degree and for preparing for meetings with your academic advisor before registering for courses.
- Accounting Advisement Form (Word document) for students matriculating Fall 2000 or later
- Finance Advisement Form (Word document) for students matriculating Spring 2006 or later
- International Business Advisement Form (Word document) for students matriculating Fall 2003 or later and list of Cross-Cultural courses (Word document)
- Management / Prelaw Advisement Form (Word document) for students matriculating Fall 2003 or later
- Managment Advisement Form (Word document) for students matriculating Fall 2000 through Summer 2003
- Marketing Advisement Form (Word document) for students matriculating Spring 2006 or later
- Minors Fact Sheet (Word document) for students minoring in Accounting, Business Administration, Economics, or Finance
If the advisement form you need is not listed above (e.g., you matriculated earlier), you will have to get the form you need in the Business Department at 119 Hartwell Hall.

Add/Drop Forms
- Add Closed Section Request Form (Word document)
Complete this form and attach a copy of your full DARS report to request adding a closed section to your schedule. - Course Withdrawal Form (opens in new browser window)
Complete this form to withdraw from a course after the drop period.
Transfer Credit Forms
- Notice of Award of Transfer Credit Form (opens in new browser window)
Complete this form to request transfer credit for a course already taken at another college. You must attach the syllabus from the course you took. - Student Course Approval Form (opens in new browser window)
Complete this form to get approval to transfer course credit for a course that you plan to take at another college. You must attach a syllabus for the course you plan to take.
Waiver Forms
- CIS106 Waiver Form (Word document)
This document explains how to apply for a waiver for CIS106 (End User Computing) and contains one of the forms you must complete and include with supporting documentation. You also have to complete a "Courses Substitution and Waiver Form" that can be obtained from the Business Department (119 Hartwell Hall) or from Registration and Records (201 Rakov Center). - ENL308 Waiver Form (Word document)
