Intake Questionnaire (PDF) - The questions asked on the Intake Form give us basic information that helps to determine how we can best to work together and also gives you the opportunity to more clearly identify your concerns. Before coming to the Counseling Center to make an appointment the form should be filled out in its entirety. HOWEVER, IF YOU ARE HAVING SUICIDAL THOUGHTS, THOUGHTS OF HURTING OTHERS, OR HAVE EXPERIENCED A RECENT CRISIS IN YOUR LIFE (e.g., death of a family member, ending of a relationship, medical problems, etc.), A STAFF MEMBER IS AVAILABLE EACH DAY TO MEET WITH YOU. .SO PLEASE FEEL FREE TO COME TO THE COUNSELING CENTER TO DISCUSS YOUR IMMEDIATE CONCERNS.
