Coordinated Care for Community Dwelling Elders

This poster identifies the problem of coordinated care for community dwelling elders. A lack of coordinated care prevents community dwelling elders from achieving optimal health outcomes, and has the potential to decrease their quality of life. This is becoming an increasingly prevalent issue, as the aging population continues to grow at exponential rates. This poster highlights the unique issues faced by members of the aging population surrounding assessment and treatment, as well as other barriers to coordinated care. Additionally, it identifies unique interventions and various geriatric models of care that are effectively addressing the issue of coordinated care in older adults. The poster also identifies pivotal resources and future directions regarding the delivery of coordinated care to older adults.
