IMMUNIZATION AND MENINGITIS MANDATES

Before classes begin, all new and re-admitted undergraduate and graduate students must:

  1. Provide proof of immunity to Measles, Mumps, and Rubella (MMR)
  2. Submit the meningitis vaccine form or the date of immunization.

These requirements must be completed before classes begin or the student will be deregistered and not permitted to attend classes. In addition, a hold will be placed on your account and the Registrar will charge a fee to re-register the classes.

Meningitis: How to meet the meningitis requirement

If you choose to receive the shot, vaccines are available at:
  • The Health Center - Please call ahead to check on availability.
  • Your health care provider – Call ahead regarding availability
  • Monroe County Health Department or call (585) 274-6151 for further information about their services

How do I get my immunization records sent to the Health Center?

Some places you might contact:
  • Your primary care physician or pediatrician. Send a signed letter requesting your immunization information to be faxed or mailed.
  • Former high school.
  • Previous colleges you have attended. They must maintain records for at least eight years
  • If you have been in the military, bring your records or inform us of your service

How do I obtain or check the immunization records that Brockport already has on file?

  • Log onto the Campus Information System (Banner)
  • Enter Banner ID and PIN number – If currently not a student, you may use your social security number and your birthdate (ex. 010175). This will prompt you to create a valid PIN number.

Measles, Mumps and Rubella (MMR):
New York State Public Health Law #2165 requires all students taking six or more credits, born on or after January 1, 1957, to provide proof of immunity against measles, mumps, and rubella.

Measles:
Two doses of live measles vaccine (administered after 1967):

  • First dose given no more than 4 days before first birthday
  • Second dose after 15 months of age and at least 28 days from first dose.
  • Combined MMR (measles, mumps and rubella) vaccine is recommended for both doses.
  • OR Date of physician diagnosis
  • OR Titer date and result – NOTE: If your titer test does not indicate immunity, you will need to receive the vaccine, per NYS law.

Mumps:

  • One dose live mumps vaccine received no more than 4 days before first birthday.
  • OR Date of physician diagnosis
  • OR Titer date and result - NOTE: If your titer test does not indicate immunity, you will need to receive the vaccine, per NYS law.

Rubella:

  • One dose of live rubella vaccine received on or after first birthday.
  • OR Titer date and result - NOTE: If your titer test does not indicate immunity, you will need to receive the vaccine, per NYS law.

Meningitis:
New York State Public Health #2167 also requires post secondary students to understand the risks and recommendations of meningococcal meningitis vaccination.
