Filing for LTD Benefits

1. Request an application for benefits from the Office of Human Resources by calling 585-395-2126.

2. Follow the instructions included with the application package. I recommend sending your completed application to the Standard Isnurance Company via certified mail.

3. Upon completion and mailing of your application, all communication regarding your case should be handled directly with the Standard Isnurance Company unless informed otherwise.
