Registration Forms
Deadline for early-bird registration is June 1, 2009.
Registration form/s may be faxed, emailed or mailed. Payment (checks or money orders) must be received no later than 6/1/09.

You will receive confirmation of your registration within 2 weeks of receipt.
Payment Method
All early-bird registration payments must be received no later than 6/1/09. Checks and money orders will be accepted. Make payments payable to: The College at Brockport – MOCS (US funds only)

Group Discount:
Special discounted rates are available for groups of 5 or more registrants. A discount of 20% percent will be applied to each registration from a single institution/agency that sends 5 or more registrants.
Cancellation/Refund Policy:
Refunds for any registration will be made a less 50% per each registration for which we receive an advance written cancellation via e-mail by June 8, 2009. No Refunds will be made after June 8, 2009. Prepaid registrants who find they are unable to attend may send a delegate in their place without penalty. Please send e-mail to community@brockport.edu.
