Course Substitution Requests:
Written approval is required before registering for a course not listed on the plan of study.
- Use this form when requesting approval to change Required Seminars
- Use this form when requesting approval to change Electives

Graduation Data Card:
- This card is required for graduation processing and degree posting. A completed form must be submitted to the Registrar's Office by midterm of final semester.

Leave of Absence:
-An approved Leave of Absence essentially "stops the clock" on the five-year time limit for program completion. This must be requested prior to the commencement of the semester in which a leave begins.

Program Extension:
- This form may be used to request an extension to the five-year time limit for program completion.
