Emergency Telephone Repair Form

This form should be used for a blue light, elevator, and residence hall entry phone that previously worked but no longer works.

Emergency Telephone repairs will be completed within 2 business days.

No charge unless you submit a repair request and the problem turns out to be that your equipment is not set up properly (i.e. - cable plugged into jack, power supply plugged into a working outlet or power strip). This charge will be $25/hour.
