Departmental Billing Number (DBN)

Work orders are usually completed within four business hours after request receipt, 8 am-4:30 pm, Monday-Friday.

A Departmental Billing Number is required for any department wishing to make international telephone calls from campus.The charges made to your "DBN" will be included in your monthly departmental billings.

Terms and Conditions:

There is no charge for the initial activation of a DBN.
No complaints for bill discrepancies will be accepted after the telephone bill due date.
No service (telephone or otherwise) is to be billed to campus telephones. These restrictions include, but are not limited to Collect, Third party, and 10-10-XXXX calls.
Departmental Billing Numbers are private and are not to be shared without authorization. Unauthorized usage of a DBN is a criminal offense. It is highly recommended that one person within each department be responsible for the discretionary distribution of this code.

Once completed, print the form.
Sign in the designated area.
