Telephone Line/Equipment Repair Form

Telephone and Telephone line repairs will be completed within 2 business days of the request receipt.

No charge unless you submit a repair request and the problem turns out to be that your equipment is not set up properly (i.e. - cable plugged into jack, power supply plugged into a working outlet or power strip). This charge will be $25/hour.

No dial tone. I can hear static on the line.  
No dial tone; buttons on the telephone set are lit. The other person hears static on the line.  
Constant busy dial tone. Telephone calls are cutting in and out.  
One Way Audio Not Ringing  
Hard for other person to hear me. Does not ring.  
Hard to hear the person on the other end of the call. Does not ring, calls forwarded to voicemail.  
Equipment Problems Dialing Problems  
Telephone set physically damaged. Dialing one number, but reaching the wrong number.  
Wall jack physically damaged. Can't make long-distance calls.  
Handset cord physically damaged (curly cord). Can't make local calls.  
Buttons on telephone set are stuck.    
Silver Satin Cord Damaged (cord from wall to phone).

Call Forward Calls not going to the queue.  
Call Pickup Phone automatically placed in "Make Set Busy" mode.

Ring Again Problem with headset. ( please check batteries )  
Speed Call Keeps dropping calls
