Attendance and Leave - Military Leave Checklist

(from Benefits Took Kit - NYS Personnel Council - applices primarily to classified service employees)

Before You Leave

Consult with HR staff to determine if it is to your benefit to use accruals before going on Military Leave at Reduced Pay.

Leave personal contact information with HR staff to ensure checks go to the appropriate person/address.

Provide HR staff with name and phone number of Commanding Officer or other contact so that HR can get orders or other pertinent information.

Consult with the Health Benefits Administrator to confirm your individual or family plan health insurance status.

Consult the Employee Retirement System at 474-7736 (Albany area) or 8-866-805-0990 (outside the Albany area) [or other retirement program in which enrolled} in regard to loan deferment or any other benefits to which you may be entitled.

Consult with HR staff to get the names/exam numbers of any eligible lists you are on.

Complete a Holiday Waiver if you are changing your current status.

When You Return

Contct HR on your first day at work to ensure your return to payroll.

Check with HR to see if exams for which you meet the minimum qualifications were given in your absence.

Contact Civil Service to request active status on eligible lists and schedule a holding of exams for which you qualify.

Contact the Employee Retirement System to see if you need to buy back any time.
