Hybrid Course Note

For fill in semester, fill in subject, course # and section # is a hybrid course that meets face to face on the following days/dates: fill in days/dates. The remaining class time will be completed using the Blackboard learning management system (LMS) and will include online readings, problem solving, and discussions. Students access this course via https://brockport.open.suny.edu using their assigned Brockport NetID. You must have access to a computer with Internet connection to participate in this course and are encouraged to familiarize yourself with the Blackboard LMS as soon as possible. Attendance at all class sessions is mandatory. Please contact the department for additional information.

100% Online, Combined (CMB) Courses

Dept. chair/director provides synchronous online days, dates, and times to R&R with schedule and/or adjustment form submission.

For fill in semester, fill in subject, course # and section # is a 100% online course, which includes a mandatory synchronous (same time) online component. Students need to be online in Blackboard (Live Chat - synchronous) as follows: fill in time, days and dates throughout the term. There are no Face-to-Face requirements for this course. The remaining course work will be completed via asynchronous course activities such as online readings, problem solving, and discussions using Blackboard. Students access this course via https://brockport.open.suny.edu using their assigned Brockport NetID. You must have access to a computer with Internet connection to participate in this course and are encouraged to familiarize yourself with the Blackboard LMS as soon as possible. For 100% Asynchronous (ASY) Courses

This is a standard course note for all 100% asynchronous courses and does not require any additional information from dept. chairs/directors.

This course is 100% online and is taken/completed via asynchronous course activities such as online readings, problem solving, and discussions using Blackboard. Students access this course via https://brockport.open.suny.edu using their assigned Brockport NetID. You must have access to a computer with Internet connection to participate in this course and are encouraged to familiarize yourself with the Blackboard LMS as soon as possible. NOTE: This course opens (“Goes Live”) one week prior to the start of class in order to give you an opportunity to get acquainted with the online environment and course requirements. Synchronous (SYN) Real Time Online Instruction Course Note

For fill in semester, fill in subject, course # and section #, students in this section join a face-to-face class via remote technology (e.g., Blackboard Collaborate or SKYPE), and will be required to attend all class meetings and complete all assignments and coursework. Students must have access to a computer with an Internet connection, a remote camera, and audio capacity to participate in this course. Students access the Blackboard learning management system via https://brockport.open.suny.edu using their assigned Brockport NetID and are encouraged to familiarize themselves with the Blackboard LMS as soon as possible. Sample course note for 100% online courses requiring proctored exams

Course Prefix/Number/Section/Title Proctored Exam Requirements***

Students are required to take a proctored midterm exam on Day/Date and a proctored final exam on Day/Date. These exams may be taken at the testing center at The College at Brockport (see: http://www.brockport.edu/support/institutional_assessment/proctor.html ). Students wishing to use a proctored testing center at a different location may do so and should contact area community colleges and four-year colleges to determine if they have proctoring services or go to: https://www.asa.stonybrook.edu/suny/eps/Visitor . Students are responsible for any associated costs and the location must have a working Internet connection. Students MUST SUBMIT A LOCATION AND CONTACT PERSON AT THE TESTING SITE TO THE INSTRUCTOR BY Day/Date. Students unable to make proctoring arrangements by Day/Date should drop this course immediately to avoid financial liability.
