Dear Students:  
  
Great news! Effective tomorrow, New York State has opened COVID-19 vaccine eligibility to everyone over the age of 16. Vaccinations are critically important to our efforts to Protect the Nest and return to normalcy in the near future. We’re trying to make the process as simple as possible by working to vaccinate you here on the SUNY Brockport campus. 

If you are interested in being vaccinated at our clinic, please fill out this survey by the end of the day on Friday, April 9. Please read the entire survey request form as it provides more detailed information than what is provided in this email.  
  
SUNY Brockport will request the Johnson & Johnson vaccine, as it only requires a single dose — increasing the likelihood that you’ll be fully vaccinated before the semester ends.  
  
If you have already received your vaccine, are in the process of awaiting your appointment for your second vaccine, have an appointment to start your vaccine regimen in the coming days/weeks or are not interested in receiving the vaccine — you do not need to complete this survey.    
  
Additionally, if you would rather obtain your COVID-19 vaccination at one of the many other vaccination sites throughout New York State, please visit the following  website  for eligibility and locations.
