Revised Direct Deposit Form

The Office of the State Comptroller (OSC) has provided a revised Direct Deposit form, along with updated instructions. The new form has been updated to provide additional employee identification information to assist in direct deposit fraud detection

OSC has provided a revised Direct Deposit form, along with updated instructions. The new form has been updated to provide additional employee identification information to assist in direct deposit fraud detection. The new form must be used and completed in full to request or make changes to your direct deposit information. If employees have saved blank copies of the old form; please dispose of them appropriately.

The new form also allows employees to opt out of receiving a paper check advice. To opt out of paper checks without making a direct deposit change, employees should sign into NYSPO, which can be accessed through the SUNY portal at suny.edu/hrportal. Click on the NYS Payroll online icon.

For assistance in completing the new form, OSC has also provided detailed instructions. The form and instructions are available through the link below.

**Please note: Employees who are already enrolled in direct deposit do not need to complete a new form unless they wish to make changes to their current direct deposit information.
