In order for dependent children between the ages of 19-25 to remain covered under an enrollees dental and vision plan, the child must be a full time student. The enrollee is responsible to submit a student attestation form confirming eligibility. Employee Benefits Division of Civil Service, has been working directly with Emblem Health (GHI) to identify children between the ages of 19-25 for which student attestation was not received.

Each month, those identified will have dental and vision coverage cancelled effective the end of the month in which the dependent turns 19, or three months after the most recent semester for which an attestation form was received.

For a dependent who is a full-time student, an attestation form must be completed and can be found on Emblem’s website (link below). This form will require the enrollee to complete information concerning the dependent student’s date of eligibility. Once completed, the enrollee will mail the attestation to the address listed on the form. After receiving this information and confirming the status of the dependent child, dependents will be reinstated (if applicable) and will receive a letter from the Employee Benefits Division of Civil Service confirming.
